What Happens in the First Week of Hospice Care?

When a loved one begins hospice, one of the most common questions families ask is:

“What happens next?”

The first week can feel like a mix of relief, uncertainty, and emotion. You may be wondering who will be coming, what support will look like, and how quickly things will change. We hope this walks you through what the first week often looks like, so you don’t have to step into it alone.

🤍 Day 1–2: Getting to Know You and Creating a Plan

The first visit is about understanding your loved one as a whole person, not just a diagnosis.

A hospice nurse will:

  • Complete an initial assessment

  • Review current medications

  • Identify areas of discomfort or concern

  • Begin developing an individualized plan of care in collaboration with the hospice physician and care team

You may also begin meeting other members of the hospice team, based on your needs and preferences, which can include:

  • Hospice aide

  • Social worker

  • Chaplain or spiritual support

There is no rush in this process. This is a time to listen, to understand, and to begin building care around what matters most.

🤍 Day 2–4: Support Begins to Take Shape

As the plan of care is put into motion, support begins to take shape in a more visible way.

You may notice:

  • Medications being reviewed and adjusted, as ordered by the hospice physician, to support comfort

  • Medical equipment and supplies arranged and delivered as needed, based on your loved one’s condition (such as a hospital bed, oxygen, or personal care and wound care supplies)

  • Hospice aides beginning visits to assist with personal care, in alignment with the plan of care

  • Ongoing communication from the hospice team to ensure care is coordinated and responsive

Everything is guided by your loved one’s needs.

Nothing is one-size-fits-all.

For many families, this is when a sense of relief begins to settle in.

💬 Ongoing Support and Communication

One of the most meaningful changes families experience is realizing they are no longer navigating this alone.

During the first week, communication begins to take shape in a steady and reassuring way.

You can expect:

  • The hospice team to begin ongoing communication that continues throughout the course of care

  • Clear guidance on how to reach the team, including after-hours support when needed

  • Questions, concerns, and changes in condition to be welcomed and thoughtfully addressed

Care is not limited to scheduled visits — it becomes a consistent layer of support, surrounding both you and your loved one.

🌿 Emotional and Family Support

The first week often brings more than just physical changes.

It can bring a quiet shift, where things begin to feel more real.

You may find yourself wondering:

  • Did we make this decision at the right time?

  • What should we expect from here?

  • How do we prepare for what’s ahead?

These thoughts are not only common, they’re part of being present and caring deeply.

Hospice care includes support for both the patient and those who love them.

Members of the care team, such as social workers and chaplains, are available — based on your preferences — to offer support, guidance, and simply a steady presence during this time.

There is no right way to feel, and no expectation to have it all figured out.
